What is an FR-19 request?

Have you received an FR-19 request from the Maryland Motor Vehicle Administration (MVA)?

An FR-19 is the name of a form the MVA requires your auto insurance carrier to submit providing proof of insurance coverage for a specific vehicle registered in the state of Maryland.  By law in Maryland, if you have a vehicle with active tags and registration, you must maintain the minimum state-required liability insurance with limits of $30,000 per person in bodily injuries, $60,000 in total bodily injuries, and $15,000 in property damage.  

Why are they requesting additional proof of insurance coverage?

Events like a lapse in coverage, policy rewrite, carrier change, vehicle total loss, or sometimes no reason at all, can trigger an FR-19 request to be sent to the registered vehicle owner.  

Is my insurance agent or company notified to send on my behalf?

Your Insurance Agent or Insurance Company is not informed when a request for an FR-19 is sent out, so please make sure you contact them ASAP to process the form.  Additionally, the MVA will not accept proof of insurance from any source other than the Insurance Company.  

What Should I Do If I Receive A Request for Proof of Coverage from the MVA (FR-19)?

Please call your agent or insurance company with the following information from the FR-19 request letter you received: 

  • Case Number

  • Title Number

  • Insurance Verification date

What happens if I’ve had a lapse in my insurance coverage?

If coverage hasn’t lapsed, the process should be pretty simple for your agent or company to resolve.  However, if you have experienced a lapse, while the MVA will assess you with a fine, it’s still best to contact us as soon as possible so we can obtain coverage for you to stop the fine from escalating. (Notice: MVA fines are subject to change; click here for the latest).
